Jimmy, your comprehensive list leaves out one BIG factor:
Upper management.
McCoy had the free reign you allude to because CBS-FM was making money that these other stations around the country didn't. Look at the annual top 10 billings - how many of these stations are NY or LA? Almost all of them - and in the late 80s/early 90s when CBS-FM was on top of the ratings, they were in that top billing as well. Which means MOST of the oldies stations you mentioned didn't make the bucks that would allow station owners to say "Go for it!" Look at WFAN - mediocre ratings but phenomenal billing... Infinity leaves them alone! When you generate the cash, you have the freedom to do almost anything with your station - including NOT changing it.
