I surmise the answer to the last question could be summed up in one word: Tradition.
KFWB Los Angeles' audio stream on the web has the TOH and BOH tone (which is set at 1 kHz, same as with WINS). However, KYW Newsradio 1060 in Philadelphia seems to have eliminated their tone, at least on the web stream, though their unique teletype SFX (sounds like a Model 14TP processed through a phone line) is still heard in the background.
